Communication at the Methodist Home

The interdisciplinary care team at the Methodist Home works closely with patients, their families, and other medical providers throughout the duration of each patient’s short term rehab stay. A few ways we make sure communication is always flowing smoothly:

  • During the admission process, patient preferences for preferred contact persons are discussed. HIPAA (Health Insurance Portability and Accountability Act) protocol is closely followed for discussing the patient’s care plan and status throughout their rehab journey with approved contacts.
  • An initial care plan is established for each patient within two weeks of admission and is accessible and followed by the entire medical team.
  • We have a unit supervisor available on each floor during the day shift, and a facility supervisor covering during evening and night shifts.
  • The Director of Social Work (DSW) and Social Worker (SW) are available Monday through Friday from 9:00 am to 5:00 pm.
  • Patient statuses and concerns are discussed among core members of the Methodist Home team during our daily morning report.
  • We have open lines of communication with our Medical Director and other medical partners, including access to the Northwell Health TeleSNF Program.
